Immunization Requirements (Kindergarten and 7th Grade): State law requires that immunizations must be completed and up to date for the student’s age OR a signed medical exemption from the child’s health care provider OR a signed and notarized conscientious objection from the parent, in order to attend school.

Failure to comply will prohibit the student from attending school. The school nurse keeps a record of each student’s health and immunization information. Any questions can be directed to the school nurse.
